Unfortunately, filling out forms is one of those unloved tasks that are necessary at the start of all treatments. Nearly all of those forms will be presented to you when you first sign in to Sessions Health. The one exception to this is the most important form of all: the intake history, which offers me all of the critical clinical information I need to get me started with you or for your child.

Below you will see links to two intake forms: one for adults and one for minor children. Please follow the directions for the appropriate form. You must then download this form to your computer and fill it out there (NOT on the browser) in any app that enables fillable forms, such as Preview or Adobe. You can take your time filling out these forms, so long as I get them before your first appointment. please remember to “save” your answers.

When you’ve completed the intake, please go onto your own designated portal on Sessions Health and upload the completed form.
